An employee was operating a pallet jack to change a forklift battery. Upon reversing, the employee's right leg became caught between the pallet jack and a scrubber, resulting in a right tibia fracture that required hospitalization and surgery.

A temporary employee was throwing garbage into a container of a truck when the driver pulled away from the loading dock. The employee fell from the dock and sustained fractures to the face, right arm, right hand, and a right shoulder contusion.

Employees were installing glass for skylights on a flat roof and were removing glass from a shipping crate. The glass shifted from its position and fell onto one of the employees, causing them to sustain a compound fracture of the right leg.

An employee had just completed installing a door header when he backed into a nail gun that was held by another worker. The employee was struck with a framing nail near the shoulder.
