An employee was cutting a 3-inch round steel rod with a band saw. Metal became lodged between the blade and the vise; the employee was working to remove it when the sawblade amputated his left ring finger and lacerated his middle and little fingers.

Employees were pulling three-phase power lines to a power pole. An employee's hand was caught between a pulley and a rope, resulting in amputation of the employee's index, middle, and ring fingertips.

An employee was using a chain fall to install a liner in a mill. As he was setting the liner in place, his right index finger got stuck in a chain on the rigging, resulting in a fingertip amputation that required hospitalization and a surgical reattachment.

An employee was replacing a water check valve at a water plant. He was lifting the valve with two hands when it slipped and contacted his right ring finger, resulting in a partial amputation without bone loss.

An employee was working to straighten a solar panel by reaching into the unguarded rotating section of the conveyor when their right thumb got caught in the conveyor, resulting in amputation of their right thumb.

The injured employee was assisting with hitching a trailer to a truck. The employee was working to engage the lunette ring on the trailer with the pintle hook on the truck. A mini-excavator began driving onto the rear of the trailer. The added weight caused the rear of the trailer to lower and the front to rise, creating a pinch point between the trailer and the truck. The employee s left thumb was caught in the latch mechanism of the pintle hook. The thumb tip was partially amputated and required surgery.

An employee was using a pipe threading machine to thread a 2-inch diameter gas pipe during the rebuilding of a gas meter. The pipe was in a vise mounted on the back of the crew truck. A few pieces of metal chips or shavings caused the threading machine to jam. The employee started to back the pipe up, but it jammed in that direction as well. While reaching to pull the handle back up into place to try and start again, the machine rotated about an inch and pinched his middle fingertip against the edge of the truck s back step. The employee's fingertip was partially amputated without loss of bone.

An employee was operating a dump truck when the trailer door swung and pinched the employee's right middle fingertip underneath the nailbed, resulting in an amputation.

On June 20, 2025, a temporary employee was cleaning the rollers on a rewinder machine when the their fingers were caught in the rollers, resulting in a crush injury and degloving of the right ring fingertip that required a surgical fingertip amputation. The incident occurred during training and the machine was in operation at the time.

An employee was assisting with an operation that was pulling in an underground wire. The employee was guiding a rope through a capstan when their gloved right hand was pulled into the capstan for approximately 1/3 of a rotation. The employee's right index fingertip was crushed and surgically amputated to the first joint.

An employee was spraying down an area by a conveyor he had been using. Moving parts (such as rollers and chains) inside a gear box amputated his right arm at the elbow. The employee was hospitalized.

An employee was adjusting a machine attached to a tractor that rolls out hose into a trench. The machine lowered to the ground and part of it landed on his big toe, amputating it.

An employee was descending a multi-position ladder when he slipped and his finger became caught in the mechanism of the ladder as he fell. The employee sustained a degloving to a finger and an amputation to the second knuckle.

An employee was decommissioning a substation. The tower switch was being set down on the ground when the switch blade moved. The employee's finger was crushed and amputated.

An employee was attaching a turnbuckle to an auger. The turnbuckle slipped and the employee's left thumb was pinched between the turnbuckle and the auger resulting in a partial amputation of left thumb.

Employees were troubleshooting the gypsum conveyors. An employee's clothes got caught in the machine, resulting in amputation of their left arm at the shoulder and amputation of part of their left ear.

An employee was switching out the metal doors on a reactor cabinet when a flash occurred. The employee experienced a 13,000-volt electric shock, resulting in head trauma and amputations of both their right and left forearms.

An employee was preparing an I-beam for blasting when the beam struck their fingertip as it was being flipped. The employee sustained partial amputation of their left middle fingertip.

An employee was de-torquing the gearbox from the main shaft bolts for the removal of the gearbox. The hydraulic 5-head torque head popped off the bolt and struck the employee's left middle finger, resulting in an amputation above the first knuckle.

An employee was troubleshooting a reverse air valve that was stuck open. The employee was working to close the valve as part of the isolation procedure. The valve closed and amputated the employee's left ring fingertip.

An employee and a crew were setting up a new utility pole. As the employee was lifting the pole to rotate it, his gloves became entangled between a chain sling and the pole, resulting in a partial amputation of the right index finger and a nondisplaced fracture of the middle finger.

An employee was utilizing a core drill to drill a foundation for a gas meter relocation. The employee's glove became entangled in the drill resulting in amputations of their left ring and little fingers.

An employee was adjusting a utility pole chain sling on a residential job site. When they stepped back away from the chain they lost their footing and the remote control for the gut line was activated. A fingertip on their right hand was pinched and amputated between the utility pole and the chain.

A crew was removing old distribution poles. As they were removing the bottom section of a pole post, a winch line became tangled with slack. Two of the employees climbed on top of the crew's truck to fix the issue. The load then pulled the winch line tight around the finger of one of the employees, causing the amputation of their fingertip.

An employee was clearing a clog in an ice blowing machine. The employee's left hand came into contact with rotary valve blades that were in operation and he sustained amputations to his middle and index fingertips.
