An employee was entering a doorway on the seventeenth floor of an enclosed building when the door closed on his left index finger, crushing it. The fingertip was amputated.

An employee walked into the back office and an altercation occurred with another employee. The injured employee was shot by a firearm, once in their side and twice in their arms. The employee's left leg was amputated below the knee and their left arm was fractured.

In the kitchen prep area, an employee was operating a meat slicer to cut chicken breast into strips. The employee went to remove a piece of chicken that was stuck between the meat grip and the bottom of the slicer tray. Their right thumb got caught underneath the meat grip and the blade of the slicer amputated their thumb tip.

An employee was working with a golf ball washer machine. He was cleaning a belt on the side of the machine using a sponge. The sponge got caught by the rotating belt and was pulled in. The employee's right hand was pulled in as well and he sustained an amputation to a finger below the nail.

On August 16, 2025, an employee was holding a door when it closed on her right little finger, resulting in a fingertip amputation halfway down the fingernail.

An employee was dislodging a towbar and hitch between two golf carts. The upper part of the employee's left index finger was pinched in the towbar/hitch, causing an amputation at the last joint.

An employee was rotating a card reprint machine on a dolly. The machine caught the employee's right middle fingertip and amputated about an inch of the finger.

An employee's hand was on a door frame when the door slammed shut. Her left little finger was amputated at the first joint.

An employee was setting up a banquet hall. While pulling on an air wall/divider their left middle fingertip became caught in the wall and was amputated.

An employee was cutting cooked chicken with a kitchen knife when the tip of their left thumb, including the nail, was cut, resulting in an amputation.

An employee was slicing tomatoes with a chef's knife when the knife contacted their little finger, resulting in a fingertip amputation.

An employee was cutting bacon when the knife he was using slipped and the tip of his left thumb was lacerated. The thumb tip was medically amputated.

An employee was preparing food with a knife. The knife cut the employee's finger, amputating it.

An employee was blocking a ham on a band saw when his right thumb contacted the saw blade. The employee sustained an amputation of the thumb tip.

An employee was washing dishes and a bottle fell behind the dish machine. The employee went to retrieve it when his finger was caught in a gear and the fingertip was amputated.

An employee was slicing radishes with a mandolin slicer when he sustained an amputation to his right index fingertip.

An employee was cleaning sections of the metal conveyor rack line. The employee picked up the conveyor rack sections and set them up perpendicular to the trough while he sprayed the food debris with the hand sprayer. There are four sections in total; after completing the first two sections, the employee lifted the third section and it fell forward. The conveyor section partially amputated the employee's left middle fingertip.

An employee was using a slicer to slice onions when the underside of his right thumb tip was severed, resulting in a partial amputation.

At about 1:00 p.m. on May 12, 2025, an employee was cutting celery with a knife. The knife severed part of the top of this right thumb, including skin and the nail.

On May 10, 2025, an employee was using a circular saw to cut 2 x 4 boards or plywood inside the engineering workshop. The saw blade amputated his right middle finger to the second knuckle and lacerated the top of his right index finger.

An employee was opening the stuck lid of an outdoor dumpster. Another employee adjusted the dumpster to get it away from another bin so the employee could open the lid. The employee's left hand was on a yellow pylon when her little finger got caught between the dumpster being adjusted and the pylon, resulting in the surgical amputation of one-third of the finger.

An employee was removing a folding cart from a catering delivery van. He was pulling the (folded) cart out of the van's side door and grabbed the bottom of the cart behind the wheel. As he twisted it to remove it from the van, he lost his grip and the cart fell. The cart opened up as it was falling and the metal bar snapped back against the top of his left middle finger. The employee's finger was crushed, partially amputated, and the tuft was fractured.

An employee was cutting sweet potatoes with a knife and amputated his left index fingertip.

On April 2, 2025, an employee was reassembling a food-sealing machine when the machine activated and a metal plate pinched their right thumb tip, amputating it. The machine was not locked out/tagged out at the time.

An employee was operating a table saw and amputated their right thumb tip.
