Two steel plates (5 feet by 10 feet by 3/16 inch) had been rigged using a shackle, sling, and tagline, then positioned vertically at the ground level by a crane. An employee detached the rigging from the crane hook and two employees were stabilizing the load as it lowered to a 45-degree angle. As the plates were being lowered, they shifted and caught the left foot of one of the two employees against stacked plates. The employee's left tibia and fibula were broken.

An employee was loading equipment onto a forklift when the shackle from the sling fell to the ground. While the employee was retrieving the shackle from under the forks, the moveable fork that was propped up on the shackle fell onto his left middle finger. The employee sustained a partial amputation to the fingertip.
