The plate was not inserted far enough, so he pushed the edge of the plate with his right index finger to reposition it. In doing so, his middle finger extended into/between the shear's point of operation as he stepped on the foot pedal actuator. The shear blade dropped down, cutting and causing a partial amputation of his right middle finger.
