Bulkhead forms were being removed from a concrete wall using a crane. An employee was about 20 feet up on rebar, prying the bulkhead loose and helping with rigging. When a form cleared the rebar, the crane began to swing. The form slid out of the sling connecting it to the crane, fell about 12 feet, and struck the employee's hard hat. The employee suffered broken vertebrae, as well as a broken ankle.
