An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

On May 14, 2024, an employee was raising the dump body on a dump truck to unload mulch. When it would not raise, the employee checked underneath the dump body to look for the issue and the dump body dropped down onto his arm, pinning it against the frame. The truck then rolled down a slight incline, dragging the employee with it. The employee's right forearm was fractured and arteries and blood vessels were injured.

An employee was standing inside the building looking out a window on the garage door. The employee knocked on the window to get the attention of employees outside. One employee that was outside knocked on the glass in response. The window shattered and the injured employee had shards of glass enter his eye, resulting in the probable loss of the eye.

On 12/5/2017, an employee was using a reciprocating saw to cut a pipe while standing on a 12-foot stepladder. While he was making a second incision in the pipe, it swung down, hit the ladder, and knocked it over. He fell and his face hit the concrete floor, breaking a facial bone and causing cranium bleeding. He was hospitalized and had surgery.
