At about 10:00 a.m. on July 13, 2025, an employee was cleaning the adhesive from running rollers using a rag soaked in cleaning solvent to prevent contamination. The rag was caught between two rollers and pulled the employee's right hand between the rollers. The employee sustained right hand crushing injuries, de-gloving injuries (palmer and dorsal sides), and fractures. The employee was hospitalized and required surgery. The machine was not locked out/tagged out at the time.

On March 21, 2025, at approximately 10:15 AM, an employee was removing an electrical feeder wire from rigid conduit using a tugger/puller. One end of a rope was attached to the wire being pulled and the other end was wound around a powered capstan (cylinder/spindle). While pulling the wire, the employee's gloved right hand became caught in the rope and was pulled into the capstan, crushing their forearm. The employee was hospitalized with fractures to their right radius and ulna, as well as the amputation of their little finger.

On October 18, 2024, at approximately 9:35 a.m., an employee was attempting to clear a clog/jam in the filling machine while co-workers were preparing to purge the machine and begin startup. While clearing the clog, the filling machine actuated and the employee's index fingertip was amputated.
