Investigation abstract
At 1:38 p.m. on March 17, 2023, two employees were sheathing the roof of a three-story townhome. Employee #1 stepped onto an unsecured platform elevated next to the eave. The platform was hooked onto the quick coupler of a telehandler and was already occupied by Employee #2. When Employee #1 stepped onto the platform, the platform tipped off its supports and dropped approximately 27 feet to the ground below with both employees on it. Employee #1 struck his head on the concrete floor within the garage and died from his injuries. Employee #2 landed on the graded soil and suffered contusions to his left leg and was treated without hospitalization.
