Employee is killed when struck by tool
Investigation abstract
At 8:30 a.m. on June 7, 2023, an employee was moving a flex stabilizer manually. While moving the flex stabilizer manually, the device was upended and struck the employee in the back of the neck/head area. The employee was crushed by flex stabilizer tool and killed.
