While walking, an employee tripped and fell over the forks on a parked pallet jack, injuring the right leg/knee.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was walking down a flight of stairs when they fell down the stairs and suffered a fractured hip.

An employee was exiting the palletizer area and stepping out into an aisleway when his left foot was run over by an oncoming forklift. The employee was hospitalized with a fractured left foot.

An employee was capturing videos at an outdoor event on July 13, 2024. They were struck in the leg by a motorcycle in a parking lot and sustained a fractured tibia.

On June 23, 2024, at 9:15 AM, an employee was walking down stairs when he missed the last stair and fell onto his left knee on the ground. The employee was hospitalized with a hematoma under his kneecap.

An employee was lowering a roller on the press machine when it slipped and rolled over his right thumb, lacerating the thumb. The employee was hospitalized and required surgery.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
