On or about January 12, 2015, an employee was injured while loosening the bolts of a rod and collar assembly on a manual assist arm. A component fell and struck his hand, amputating the tip of his left little finger.

An employee was working inside an engine index cell and was standing in a pit approximately 2.5-3 feet below ground level. The employee went to move from the inbound part of the cell pit to an area between inbound and outbound. He stepped up and out of the pit, over guiderails, and onto a roller ball surface. His foot caught on the guiderail and he tripped and fell forward, fracturing his femur.

On or about March 12, 2016, at approximately 2:00 AM, an employee was working on the assembly line, attempting to stow a steering hose, when her hand was caught between machine components. Her ring finger was amputated at the distal tip, requiring hospitalization.

On February 17, 2016, an employee was installing a fascia and fog lamp on a car. As he was pulling on the wire to connect the fog lamp, he struck the car and punctured his right hand between the index finger and thumb. The injury became infected and required hospitalization.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

International Steel and Counterweights LLC

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

An employee was using a battery-powered torque tool to fasten a bolt. The tool was activated and the socket of the tool rotated and pulled in their gloved left hand. The employee sustained a left index fingertip amputation.

An employee was inspecting a casted part that weighed 330 pounds. The part fell from a lift assist onto the employee, striking his upper right arm and head. He suffered burns to the upper right arm and was hospitalized.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
