An employee was working to assess the operation of a high-pressure water nozzle. The water system was activated and the employee was cut by high pressure water on his left hand where the palm meets the wrist. The employee was hospitalized.

An employee's left bicep was lacerated by high-pressure water during water blasting operations. The employee was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.

An employee was sandblasting paint from a water tower when the blasting media sprayed across their forearm, resulting in an elbow injury that required hospitalization.

An employee was airing up a pipe plug to perform a test but was encountering an error. He entered the manhole to ensure the hose had not fallen off or determine any other visible issues. While he was in the manhole pressure testing, the air bag blew out of the line, hitting his left leg and resulting in a broken leg and a laceration to his chin.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.

An employee was removing a roll (tooling) from a machine when their left little finger was pinched between the roll and the machine, resulting in a partial amputation.

An employee was rolling a piece of steel in the belt of a shape roller machine when his finger was pinched between two pieces of steel. His right ring and middle fingers were amputated.

An employee was cutting a length of steel coil when he noticed the steel might have cross-break defects. He used his gloved hand to feel for and clean the defected spots when his glove got caught in the measuring wheel, which rolls along the top of the steel to ensure accurate cuts. The employee's arm was pulled down into the machine until his body was stopped by a support bar that goes across the machine. The employee's arm was fractured at the elbow and required hospitalization.

Two employees were loading a roll into an EDT machine. One employee was operating the crane and the other employee was the EDT operator. The crane operator was between the roll and the EDT machine. Both employees were struck by the suspended roll and knocked into the EDT machine wall. Both employees sustained internal abdominal injuries requiring surgery and hospitalization. One employee was fatally injured.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

An employee was walking through the dining room when she slipped and fell to the floor, resulting in a fractured left femur.
