Federal OSHA recorded a severe workplace injury at Cintas Corporation, 5100 26th Ave., ROCKFORD, ILLINOIS 61109 on — Chemical burns and corrosions, unspecified, affecting the multiple face locations.

On or about January 13, 2015, a maintenance employee of Cintas received a chemical spray of sodium hydroxide to the eyes and face requiring hospitalization. The incident occurred during offloading of the sodium hydroxide from a contracted delivery service's truck (Hydrite).

An employee stepped up on the back of the work vehicle which was parked at a customer parking lot and began manually unloading a 4 galvanized pipe from the ladder rack on top of the vehicle. While removing the pipe, the employee lost his balance and fell backward off the back of the vehicle. The pipe that the employee was unloading was pulled down as he fell. Upon landing on the concrete parking lot, the pipe pinned the employee s right hand against the concrete. The employee sustained a fracture and laceration to their middle finger that resulted in medical amputation of the finger.

An employee was standing at her work station, folding towels, and passed out. The employee fell to the floor, hitting her head. The employee was hospitalized with a laceration to the head.

An employee was walking to the time clock at the end of her shift, tripped over a mobile clothing rack and fell breaking her scapula and three ribs.

An employee was working on an industrial washer. He was trying to get the drum to rotate when it moved unexpectedly, pulling his finger into the nip point of the belt and pulley. It then rocked back, discharging his finger from the nip point. The employee's pinky finger was amputated. The machine had been locked out at the time of the incident.

On November 27, 2023, an employee was emptying a 2-inch product line that transports sodium hydroxide liquid from a rail car to a 275-gallon tote tank. The nozzle came out of the tote and sprayed sodium hydroxide onto the employee's face. The employee was hospitalized with chemical burns to their face, mouth, and neck.

JOST CHEMICAL COMPANY, INC.

An employee was working with sulfuric acid as part of the production process. While transferring the chemical from a large container to a smaller container, it splashed on his body and hand, resulting in a chemical burn.

An employee knelt in wet concrete while performing work as a concrete finisher and sustained a chemical burn to the right shin. The employee was hospitalized and required surgery.

An employee was using a 5-gallon bucket to unload acid product from a tank. Residual product leaked into the containment area, causing the employee to sustain first- and second-degree burns to the chest, as well as third-degree burns to the arms.

An employee was transferring an alkaline cleaning chemical from a bulk container into 1-gallon containers. The employee lifted a gallon container by its label tag. The tag broke causing the container to fall approximately 14-18 inches. The container struck the ground and the contents splashed onto the employee causing chemical burns to their eyes.

A maintenance technician was investigating a noise coming from a conveyor. His left hand was caught by a roller and pulled in up to the forearm. The employee's forearm was fractured.

An employee was preparing a truck at a loading dock. She was placing the manual dock leveler device when her left middle finger became caught in the leveler. The employee's finger was partially amputated.

An employee was using a machine that irons and folds table napkins when a string from her sweatshirt got caught in the rollers. Her hand was pulled into the machine's point of operation, resulting in crush injuries that included a broken wrist.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
