An employee was arranging stock on shelving units while on an A-frame ladder. The employee was descending the ladder when she fell to the ground. She was hospitalized with a fractured right tibia.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was cutting pieces of plywood with a table saw and amputated their left thumb.

An employee was operating a standing forklift and tried to prevent boxes from falling. His left little finger was crushed between the forklift and metal shelving resulting in amputation.

An employee was driving a reach truck down an aisle when the truck collided with a structural column. The employee's arm left the operator's compartment, and the employee suffered a broken arm.

An employee was working at the cash register. When they bent down to tie their shoe, they lost balance and fell to the floor, resulting in a broken tibia.

On September 24, 2021, at 8:50 a.m., an employee was driving a stand-up forklift when the forklift collided with palletized material. The employee's left foot was crushed under the lift, resulting in multiple toe amputations.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
