Employee was using a Bobcat with a jack hammer attachment to flip a metal caster lid (that weighs approximately 300 pounds). When the employee flipped the lid, the edge of the metal casting lid hit his leg, injuring him while he was in the Bobcat.

An employee was being trained to operate a machine. He was running copper tubing into the machine when the crimper was activated and his left thumb became caught in the crimper resulting in partial amputation of the thumb tip.

An employee had just replaced a hydraulic hose on a machine and was checking for leaks. The machine cycled as the employee's right hand was resting at the ram. The employee's index finger was amputated at the first joint. The ram was not guarded at the time.

InterCon Construction, Inc.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

RAVA Construction, LLC

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was polishing a bar on the lathe with sandpaper when the bar wrapped around the employee's glove and pulled it off. The employee sustained fractures to their index finger, wrist, and a knuckle.

An employee was using an electrical meter to troubleshoot an electrical drive system. While removing the probe, the employee made contact with the bus and was shocked. The employee sustained burns to their fingers and a dislocated shoulder.

An employee was using a metal bar tied to a nylon strap attached to a jib crane to tighten a metal ring on a heated extrusion press. The metal bar flew off and struck the employee in the left forearm, fracturing it.

An employee was troubleshooting the lift table of the automated take-up mechanism on the jacket extrusion line. He was able to get the table to raise up, at which point it engaged and lowered. The employee's leg was pinned between the lift table and the concrete floor. His ankle was broken and he was hospitalized, requiring surgery.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

Hobby Lobby Stores, Inc.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.
