While lifting a pallet of cases, three cases fell off the pallet. The forklift operator picked up two cases and stacked them on the dash of the standing forklift. When he went to pick up the third case, the other two cases fell over engaging both the dead man's switch and the joystick, causing the forklift to rotate pinning him against an adjacent forklift.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.
