On November 17, 2016, an employee was operating a napkin printing press and noticed a defect. The employee turned the machine off and felt the printing plate to see if it was lifting off the cylinder. The jog button was pressed while his right hand was feeling the plate and his hand was caught between the plate and the impression roll. He suffered an amputation of his right thumb pad and laceration to his right index finger.
Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.
