An employee was walking in the hallway when she tripped on a mat. She fell to the floor, hitting her arm on the wall on the way down. She fractured her right arm.
An employee was repairing cable lines at the top of a telephone pole. As he was climbing down the pole using the preinstalled pegs on the pole, he missed a step and fell 10-12 feet breaking both legs and an ankle.
On or about 6/7/2016, at approximately 1425, an employee was working from a ladder to access a cable TV lock box when the ladder kicked out, causing the employee to fall. As he fell, he struck a stairway railing and injured his left wrist/hand and ribs.
