An employee was hospitalized for dehydration after a service call.

An employee was repairing cable lines at the top of a telephone pole. As he was climbing down the pole using the preinstalled pegs on the pole, he missed a step and fell 10-12 feet breaking both legs and an ankle.

On or about 6/7/2016, at approximately 1425, an employee was working from a ladder to access a cable TV lock box when the ladder kicked out, causing the employee to fall. As he fell, he struck a stairway railing and injured his left wrist/hand and ribs.

During a sales blitz activity, an employee participated in one of the 'minute to win it' games. The game involves a small Kleenex box with balls in it and a ribbon that goes around the waist, and the player has to twist their waist to get the balls to come out. As the employee twisted her waist, she injured her knee.

An employee was working on a residential cable connection outside while standing on an extension ladder. While the employee was descending, the base of the ladder slipped on ice, causing the ladder and employee to fall. The employee suffered lumbar and hip injuries.

An employee was transitioning from a ladder onto a roof when the ladder slipped and they fell approximately 20 feet to the ground. The employee was hospitalized with fractures to their pelvis and heel.

The scissor lift guard rails were below the girder of the crane to avoid contact. The tow arm for the crane collectors contacted the guardrail of the scissor lift and caused it to tip over. The employee stayed inside the scissor lift basket as he fell to the floor. The employee sustained fractures to the right arm and hip.

An employee was on a forklift that was elevated approximately 20 feet and was auditing inventory. The forklift malfunctioned and the employee fell down to the surface below, causing them to sustain multiple fractures.

An employee in an aerial bucket was attaching a cable to a pole when they contacted a power transmission fuse and sustained burns to their left hand, arm, and shoulder.

At 2:30 p.m. on February 8, 2024, an employee fell while installing cable, landing about 20 feet below and suffering a broken arm. The employee was hospitalized.
