A vessel was being unloaded with steel pipe. A forklift was coming in to pick up material to be put into a truck. An employee was inspecting the load that was to go into the truck. As he was looking at his check sheet, the forklift operator made a 90-degree turn and backed into him, injuring his right leg and foot.

Two employees were breaking out pipe for loading and transport at a shipyard dock. One of the breakout straps broke, causing the pipes to roll and strike the employees' legs. The first employee suffered a broken right ankle, and the second employee suffered a contusion on the left leg and a bruised right leg. They were both hospitalized for treatment.
A load of steel pipes was being lowered into a barge. The employee attempted to direct the load of steel pipes and his right arm and hand were pinned between the barge and the steel pipes. He sustained multiple fractures.
