The injured employee had his left hand on a forklift, signaling to the operator to lower the forks due to an unbalanced situation with a parts catcher screen assembly. In the process of lowering the forks the screen assembly shifted smashing the tip of the employees index finger on his left hand between the bonnet of the manual valve of the screen assembly and the forks.

While on top of the trailer, the employee stepped backward and fell off the trailer to the ground, landing on his back. The employee was hospitalized for a fracture of their C5 vertebra.
