An employee was inspecting counter weight cylinders for clearance when the employee fell from the top of the counter weights to the crane deck, suffering a facial fracture.
A FESCO employee was on top of a trailer assisting a forklift operator with placing equipment onto the semi-truck trailer. While on top of the trailer, the employee stepped backward and fell off the trailer to the ground, landing on his back. The employee was hospitalized for a fracture of their C5 vertebra.
Three employees were breaking a threaded tool string connection using two 36-inch pipe wrenches. One of the pipe wrenches slipped and struck an employee in the abdomen, resulting in a ruptured spleen.
