On or about 2/05/2015 at approximately 10:15 a.m., an employee was delivering merchandise to a customer. When the employee was placing a case of bottled soda on the hand truck, a couple of glass bottles fell to the ground and the employee was struck with glass fragments in his left eye. The employee was hospitalized overnight for glass extraction.

An employee was traveling through a warehouse with a pallet jack. The pallet jack slipped on a substance on the floor and ran into a horizontal rack beam. The employee's foot was caught between the beam and the pallet jack, and he suffered a laceration to the ankle as well as tendon/ligament damage.

An employee was traveling across a dock on a pallet jack and turning down an aisle. The vehicle struck a rack, causing the employee's foot to become stuck between the rack and the pallet jack. They sustained a laceration to the bottom of the foot and two fractures.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

RMVK Enterprise, INC.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

On October 16, 2025, an employee operating an order selector was passing a moving scrubber when the scrubber suddenly stopped. The order selector struck the back of the scrubber, resulting in a cut to the employee's left ankle.

On September 5, 2025, an employee was walking in the parking lot to get product off the dock. A 26-foot box truck was backing up and pinned the employee against the dock plate. The employee sustained fractures to three vertebrae in their back.

UNITED NATURAL FOODS, INC.

On August 30, 2025, an employee was operating an electric double-pallet jack. He was coming out of the new extension through the separation curtains when his pallet jack struck an electric forklift. The forklift landed on the employee's right foot, fracturing the big toe and second toe.

An employee was changing brake pads underneath a trailer unit. They used a hydraulic floor jack under an axle to secure the trailer in an elevated position. The trailer rolled and collapsed while the employee's head was underneath the air bag plate, resulting in a concussion, a laceration to the left side of their face, and a bruise on the right side of their face. The employee was hospitalized.

An employee was pushing an empty onion cart when his left little finger was crushed between the cart and a meat rack. The finger was partially amputated.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
