Employee was clearing a jam on a hydraulic press. During the process, the hydraulic system activated, and the ram caught the employee's right wrist and pinched it against the inner frame. The employee fractured his right wrist.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Netzsch Premier Technologies

An employee was standing on some large wooden storage crates, adjusting a tarp to cover them. He fell from the crates to the parking lot ground, landed on his left side, and suffered several broken ribs. He was hospitalized.

Cincinnati Incorporated

An employee was remotely operating a 25-ton gantry crane with a magnet attachment to unload a steel plate (approximately 96'' x 280'') from a semi-truck to the ground in the steel yard. The plate struck the employee as it was being moved, resulting in three fractures to his right arm that required hospitalization and surgery.

Klein Tools, Inc

An employee was polishing a guide pin using a CNC lathe when his left glove got caught on the chuck or tool and three of his fingers were pulled in. The employee was hospitalized and underwent reattachment surgery for finger amputations.

ALM Positioners, Inc.

On May 2, 2025, two employees were lifting a pallet jack with a built-in weight measuring plate into the bed of a truck when the plate shifted as they were lifting it. The plate crushed and severed the injured employee's middle and ring fingertips. A fingertip was amputated.

LMT Onsrud Management Inc.

An employee was cleaning a camera lens on a wheel dresser, a machine used to correct the shapes of grinding wheels. The employee's right index finger was caught between the wheel dresser and the grinding wheel being serviced. The fingertip was amputated at the first joint.

JERSEY CITY MEDICAL CENTER

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
