An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

General Shale Brick, Inc.

An employee was operating a slab line in a plant that produces 16" x 9" x 3" concrete slabs that weigh approximately 25-30 pounds. The employee went to adjust the chain and sprocket for the doubler that stacks the slabs two high, when their right middle finger was caught in the machine. The employee sustained a partial transphalangeal amputation of the finger.

An employee was packaging brick. A brick broke from the bundle and became lodged between the conveyor rollers. The employee was removing the brick when the bundle of brick shifted forward, catching his left index finger between the bundle and the conveyor roller and resulting in a partial amputation to the fingertip.

An employee was observing the operation of the stacker machine and climbed onto a platform to align bricks in the stacker. While the employee was on the platform, the machine cycled, striking and pinning him to the chain conveyor. The employee suffered abrasions on his arms and hairline fractures of his pelvis.

The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
