An employee was testing the back-up alarms on a truck in a maintenance shop. A chock on a wheel failed and another chock slid. The truck rolled backward and caught the employee's right leg against the bumpers of a parked utility truck. The employee suffered a broken right femur and was hospitalized, requiring surgery.

A welding technician was installing hydraulic packing cylinders for an Automate Side Loader (ASL). The cylinder was on a stand. When the employee attempted to move the cylinder it slipped and fell on the employee's finger pinning the employee's hand against the front part of the truck body. When the employee tried to remove their hand from under the cylinder, their right ring fingertip was amputated.

D&K Welding Services Inc.

An employee was installing decking on a roof when he attempted to cross over to the roof of an adjacent building. He fell approximately 30 feet to the ground between the two buildings, puncturing his lung, cracking two ribs, and shattering a pelvis. He was hospitalized.

Employee was installing a new cable on a home while working from a Lunar, 28 foot fiberglass extension ladder. A gust of wind made his ladder move and he felt like it was going to fall so he jumped off the ladder toward the grass to avoid the concrete patio and landed on his back.

An employee was stepping down from his truck when he slipped and fell, twisting his ankle. The employee's ankle was fractured and he was hospitalized.

An employee was working on a loading ramp that was stuck open (upright). The employee went under the ramp to release it. When he pulled on a chain that was holding the ramp open, the ramp came down rapidly, striking the right side of his face (cheek) and the right side of his neck. The employee was hospitalized.
