An employee was on a 12-foot step ladder changing a light bulb in the facility. Another employee drove his vehicle into the building and bumped the ladder; the injured employee fell off and broke both ankles.

An employee was going to inspect a gas line for damage after the gas line was struck during excavation. The employee was looking into the excavation site when he fell 3 feet into the trench and sustained a left wrist laceration that required surgery and hospitalization.

An employee was descending a step ladder when he contacted a pry bar that was in the ground next to the ladder. The employee sustained a laceration to the leg and buttocks.

An employee was helping to reposition a newly set pole that was being held by a digger derrick. While the winch line and choker chain were under tension, the employee started to rotate the pole. A link of the chain pinched the employee's left ring finger against the pole. The finger was fractured and the fingertip amputated, but was able to be reattached.

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.

An employee was unwrapping a pallet of empty buckets. The employee stepped backward and was struck by a powered industrial truck carrying a pallet of bucket lids, resulting in a left ankle fracture.

An employee was walking in the racking area to check parts when they were struck by an oncoming forklift. The front wheel ran over the employee's right leg, resulting in fractures to the leg and ankle.
