An employee driving a tractor with an attached dirt pan struck another employee who was driving a tractor with an attached dirt pan. The employee was thrown out of the back window of the tractor and landed on the ground unconscious, resulting in hospitalization.

An employee was traveling down an aisleway in a three-wheeled golf cart. They were moving around a parked forklift when a material cart came from the opposite direction. A wheel on the material cart caught a piece of racking causing the material cart to swing out and hit the injured employee's golf cart. The material cart continued to swing, hitting the injured employee's left leg. The injured employee was also knocked out of the golf cart and landed on the ground 2-3 feet away. The injured employee sustained fractures to the left leg, requiring hospitalization and surgery.

An employee was riding on a flatbed trailer when his left foot was caught between the trailer and a trackhoe. He sustained a laceration to his left foot that required stitches and hospitalization.

Menzies Aviation

Employees were operating a baggage tractor on a service road. The tractor made contact with an aircraft's left wing and the seat of the tractor was ripped off. The employees were thrown to the ground. Two employees were hospitalized with injuries including a chipped tooth, bruises and abrasions, a dislocated hip, a fractured humerus bone, and a laceration to the left knee.

Expressjet Airlines

An employee was driving his vehicle (golf cart style) to an aircraft when he passed by two large baggage carts and was T-boned by another vehicle. He was hospitalized with a broken left leg.

An employee was operating a forklift, which had a T-bone collision with another forklift. The employee suffered four broken right ribs, a bruised right lung, and skin bruising and was hospitalized.

Stevenson Crane Service, Inc.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

Severo Ortiz Construction LLC

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

Fence Repair Company

An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.

Beth-Allen Scaffold & Equipment

An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

Johnson Industrial Coatings, LLC.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
