Final narrative
On March 20, 2015, at approximately 11:00 p.m., an employee was using a core drill to remove anchors from the face of the Bluestone Dam in Hinton, West Virginia. A 1/4" to 3/8" rope became entangled in the 3" core bit. The employee's left arm was pulled into the rotating bit, which resulted in fractures of the radius and ulna.
On January 21, 2015, a tag line became entangled around an A-frame metal beam weighing approximately 1400 lb., causing the beam to fall against the leg(s) of a worker. The worker suffered fractures of the right lower leg and was hospitalized.
An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.
An employee was grinding some hardfacing on a bulldozer blade when the 9-inch grinder kicked back and lacerated his left leg above the knee. The employee was hospitalized and required surgery.

The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.
