An employee was walking from his office to his personal vehicle in the parking lot. As he descended some wooden steps, he slipped and fell down 3 to 4 stairs to the asphalt parking lot, tearing the quadricep tendons in both of his knees. He was hospitalized and had surgery.

An employee was cleaning a cordless drill. As they applied brake fluid to the drill and pulled the trigger, the fluid ignited. The employee suffered burns to the face, hands, and arms.

An employee was welding on a rack when sparks ignited residual diesel on his pants. The employee sustained first-, second-, and third-degree burns.

An employee was spraying glue to the floor of an enclosed cargo trailer. The fumes made contact with the heater causing a flash fire. The employee suffered burns to their face and right hand.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder.
