An employee was injured while troubleshooting a stacker machine. He fell and his left hand entered the press cavity on an 800-ton press, injuring the third and fourth fingers on his right hand.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

The machine was not guarded at the time.

An employee slipped on ice in a parking lot and fell, suffering five broken ribs. The employee was hospitalized.

An employee was working at the carve station when his left middle finger was caught in the pinch point between the rollers and the wheel/steel plate resulting in a fingertip amputation.
