On or about 4/1/2015, at approximately 9:30 a.m., a maintenance employee received an amputation injury to his right thumb while operating a lathe. The employee was using an Emory-type cloth to smooth a part in the lathe when his glove got caught on the part, cloth, and/or machine, causing his injury. He was hospitalized.

An employee was running a high-pressure water cabinet waterblast unit. When the employee released the wand, it propelled itself out of the blast cabinet. The wand whipped around and the high-pressure water stream lacerated the employee's upper right arm.

An employee was operating an impact sander when their hand contacted the sanding belt, resulting in amputations to the right little and ring fingers.

An employee was troubleshooting and conducting maintenance on a wax injection molding die. An actuating part caught their left index finger and amputated the fingertip.

An employee working in the foundry started feeling dizzy around 2:00 PM. Around 5:00 PM, the employee began to perspire a lot and went to his car to lay down. His arms were seizing and he had leg cramps. The employee was hospitalized due to heat exhaustion.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

Alsco Inc.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Signicast LLC

An employee was using a grinding machine with a foot pedal. She was loading a part into the grinder when her right hand got caught in the grinder. She then used her left hand to pull out her right hand. The employee suffered a laceration to the back of two fingers on her left hand and her left forearm, as well as partial amputations to two fingers on her right hand.

Oklahoma Investment Casting Company

An employee was looking into a furnace to check for parts. When the furnace closed, the employee's right little finger was caught by a piece of metal, resulting in a partial amputation.

Stephens Pipe and Steel, LLC

A forklift was loading bundles of pipe onto a trailer. One of the bundles weighing approximately 1,200 pounds dislodged and fell off the trailer, striking two employees that were standing on the back side of the trailer. Both employees were injured. One employee was struck in the legs, sustaining fractures in both legs. His left leg had to be amputated above the knee on 5/2/2025. The second employee sustained multiple injuries including a broken pelvis and kidney damage.

Accurate Specialties Inc

An employee was putting a metal part into a CNC lathe when the part fell on their right fingers. The employee sustained a fracture and laceration to their middle finger, and an amputation of their ring finger.

Sturm, Ruger, & Company, Inc.

An employee was dislodging a part that was stuck in the shot blasting machine when the part released and shifted, catching the employee's left middle finger between the part and the belt of the machine. The employee pulled his hand away and sustained a fingertip amputation.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
