Final narrative
On or about 4/1/2015, at approximately 8:30 a.m., a PIV ran over an employee's foot. The injured employee was talking with the PIV operator about a process. After they finished taking, the PIV driver began to turn and reverse and ran over the employee's right foot. The employee was hospitalized overnight.

An employee was operating a forklift. His foot was outside the tow motor when his pinky toe became caught on a walkway barrier. His pinky toe broke, requiring hospitalization and surgery.
