On or about April 3, 2015, at approximately 10:30 p.m., an employee was operating a pallet rider in the warehouse when his ankle got caught between the pallet rider and a post. He suffered a left ankle fracture and was hospitalized.
An employee was on a step ladder getting shoes from the top shelf. She fell off the ladder approximately two feet and sustained a broken left tibia that required hospitalization and surgery.
