Final narrative
An employee was attempting to repair a hole in the roof of his commercial trailer (van). In order to do so, he leaned a ladder against the wall inside his trailer. The ladder slid and he fell off the ladder fracturing his right arm and bruising his left knee. He also lost one tooth and required stitches on his lips.
