A maintenance employee was troubleshooting a cab assembly. He put his arm between a positive stop and the guide assembly. The cab assembly was not locked out or blocked, and the assembly dropped, hitting his left forearm. The arm was broken and the employee was transported to a local hospital and admitted.

An employee had just put trash into the compactor and stumbled on an uneven floor surface causing the employee to fall. The employee suffered right-side injuries to the hip, shoulder and elbow.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

Owens & Minor - Pittsburgh Distribution Center

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was operating a cherry picker in a warehouse when they fell from the picker to the floor, resulting in a brain injury and a broken hip.

An employee was performing a paper roll change at a paper sheeter machine. He was turning a non-driven unwind stand and the roll by hand when his left little fingertip got pinched between a machine depression in the chuck and the sensor. The employee's fingertip was partially amputated.

An employee was pulling an order from the third level of a warehouse racking system and fell 15-20 feet from the forklift to the ground. The employee sustained fractures to the left tibia and fibula, left shoulder, right elbow and nose. The employee was wearing a safety harness but was not tied off at the time.

An employee was setting up a machine when their left glove was pulled into a nip point. The employee's finger was pulled in and they sustained a partial amputation.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

United Refrigeration Inc

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
