An employee was injured while welding a joint on a field plate. When the joint was re-lifted, the weld broke, and a field plate landed on his left foot. The resulting fractures required surgery.

An employee was sandblasting at a bridge when the nozzle and blast tip of the line unthreaded. The employee fell backward injuring his back and metallic grit (sandblasting material) caused an abrasion to his arm. The employee was hospitalized.
