A welder was using a 4 inch handheld carbon tooth cut-off saw to cut an aluminum block off a Bradley vehicle when the saw bounced back and hit him approximately 2 inches above his right knee causing a severe laceration. The employee was taken to the hospital for surgery to repair the injury.

An employee was walking when his foot caught residual adhesive from floor tape and he tripped and fell to the concrete floor, resulting in a fractured right hip.

An employee was using a powered pallet jack to move a fixture. His leg became caught between the pallet jack and the fixture and he fell on the same level, resulting in a fractured left fibula.

The injured employee was trying to hold a door open for another employee when the automatic door closed on the injured employee's left ring finger. The employee's fingertip was amputated halfway down the fingernail.

An employee was inside a mid-level ballast tank onboard an amphibious transport dock (naval vessel). He was walking across the deck and stepped onto a board that was covering an open hole. The board split and the employee fell about 15 feet to the bottom of the tank. He sustained internal bleeding, a brain hematoma, and fractured the C6 and L1 vertebrae.

While adjusting the rigging for the bull gear installation, the bull gear became unstable and shifted. The shifting gear contacted the mobile platform on which the employee was standing knocking the platform over and causing the employee to fall to the ground. When the gear shifted, the employee's right index finger was caught between the rigging and the wood block resulting in amputation of the finger.

An employee was using a saw to cut a piece of concrete when the piece gave way and pushed the saw into the employee's leg. The employee's leg was lacerated, requiring stitches and hospitalization.

An employee was operating a rotating tool to place thread protectors on the end of pipe. Her left hand was on the rotating part of the tool when the tool engaged, resulting in a left forearm fracture and a thumb laceration.

On November 7, 2024, at about 12:30 PM, an employee was moving a cannon on a gantry crane when the straps holding the cannon broke and it fell. The employee was hospitalized with a crushed pelvis, multiple fractures, and other severe injuries.

An employee in the welding training booth was working to back gouge a weld on a 5mm plate. While grinding, the gouger high-centered and walked over the plate striking and severely lacerating the employee's left thumb.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

McAneny Brothers, Inc.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
