Final narrative
An employee was moving an aerial lift. He was attached via a lanyard when the aerial lift basket bounced, catapulting the employee. The employee was still in the basket after the accident occurred. He injured his left knee and fractured his left leg (left open fibula fracture). He was hospitalized.
An employee was replacing the battery on a paver when he shut the cover of the battery compartment and his left finger was pinched in the cover. The employee sustained an amputation of the fingertip.
An employee was using a forklift to stack hay bales when some hay bales fell onto the cage. The employee was ejected from the forklift and landed on the ground, resulting in head and neck trauma that required hospitalization.
An employee was using a powered pallet jack to move a fixture. His leg became caught between the pallet jack and the fixture and he fell on the same level, resulting in a fractured left fibula.
