Final narrative
Two employees were filling a mixer with ingredients. They were each pouring over magnets from two locations on the same mixer. The first employee picked up his magnet and walked past the second magnet as they prepared to close the mixer. As the second employee closed the mixer doors, the two magnets were attracted together. The first employee's fingers got pinched between them. The injury resulted in amputation of two of his fingertips on 5/11/15.
An employee was offloading containers from his truck using a deck that goes up and down. When the deck was up, he took a step and his foot went between the truck and the deck, resulting in a foot/ankle fracture.
