The employee was part of a scaffold construction crew. Immediately prior to the incident, the employee had been conversing with another crew member while in the process of looking for an electrical cord. The employee became very pale and sweaty and suffered an apparent heart attack.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
