An employee was putting air in a tire when the tire blew off the machine it was on. The employee was struck by the tire and sustained a right forearm fracture and injuries to the right eye and ear.

An employee had just finished servicing a belt on a vehicle. As he was inspecting his work, his hand contacted the belt and the tip of his right index finger was amputated.

On May 26, 2018, an employee was removing a truck tire from overhead. The tire fell, bounced on the ground, and hit him on the shin. He was hospitalized and underwent surgery to remove a severed internal artery.

An employee was doing a wheel alignment. He stepped out of the vehicle, fell to the ground, and lost consciousness.

An employee was performing maintenance on conveyor lines when he jumped out of the way of a moving rail-guided vehicle (RGV). He landed in the conveyor line and was then bumped by the rail-guided vehicle, causing him to strike an angle iron that was a part of a stationary conveyor. His stomach was crushed and the employee sustained a laceration to the abdomen.

An employee was troubleshooting a hydraulic piston on the wheels of a semi-trailer dump truck. The piston released and caused the trailer to move, crushing the employee's leg. The employee sustained fractures and contusions.

An employee was working on the attachment securing mechanism of a skid steer when their hand was caught by the mechanism. The employee was hospitalized with fractures and lacerations to three fingers on the left hand.

An employee was spreading the forks on a parked forklift when he was struck in the face by one of the forklift tines, resulting in a contusion.

An employee was releasing a chain to open the rear flap of a dump truck when three of their fingers got caught between the chain and the truck, resulting in crush injuries.

An employee was carrying a plastic tote full of brake rotors. His foot caught the corner of a pallet, causing him to fall to the concrete floor and break his hip.

An employee tripped on a rotor on the floor of a store, fell, and suffered a broken right thumb.

An employee was working on a vehicle in a bay. As he turned around, his foot caught on the leg of a lift and he heard/felt a snap and pop in his right hip and fell. The employee sustained a torn hamstring in his right leg.

An employee was installing a driveshaft on a customer's vehicle. He was using a floor jack to hold the driveshaft in place to line up the bolts. The driveshaft moved off the jack and crushed his left ring fingertip, resulting in an open fracture of the tuft of the distal phalanx, as well as a partial amputation.

An employee was in a truck, helping a team lift a 55-gallon drum into the truck. The employee fell to the ground and suffered a broken left arm. The employee was hospitalized and required surgery.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
