An employee was installing a joist seat at roof level while standing on an extension ladder that was placed on metal decking. The ladder began slipping and the employee fell approximately 7 feet to the ground surface. As he fell, his foot was caught between the rungs of the ladder. The employee sustained a broken right ankle requiring hospitalization and surgery.

On January 26, 2023, at 11:30 AM, an employee was installing a brace in the top slot when the brace jumped, causing the employee's hand to become lodged between the column and concrete wall. The employee sustained partial amputations of the thumb, index, and middle fingers at the first knuckle.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf.
