An employee was working in the aisle stocking items when a forklift was leaving the receiving department. The forklift hit the employee in the stock aisle and pinned them against the shelving unit. The employee sustained a broken ankle and a ruptured liver.

To help move cardboard boxes, an employee used a lift machine to move the pallets storing the boxes. He exited the lift while it was still in motion and his hand was caught between the back of the lift and a yellow bin. He was hospitalized with a broken/bruised right thumb/palm.

An employee was operating a reach forklift in an aisle that had been blocked off with a safety barrier. A customer went under the safety barrier and walked in front of the forklift to grab a case of mayonnaise. The employee tried to step off the forklift, but his shirt got stuck on the drive handle. The forklift then accelerated into steel pallet racking and the employee was pinned between the rack and the forklift. The employee sustained a fractured pelvis.

An employee was operating a picker when it struck a stack of pallets. His body moved forward and he struck his face on the computer monitor in the vehicle, resulting in a concussion, laceration, and blurry vision.

An employee was driving a lift in reverse through an aisle while the lift was loaded with a headboard. The headboard caught merchandise in the aisle racks, causing it to shift and contact the employee's right leg. The employee suffered a broken leg.

An employee was driving a utility task vehicle (UTV) through the parking lot to the boat yard to meet a co-worker. The UTV and slid into a curb, causing it to flip on its side and land on the employee's leg, resulting in a broken leg.

On December 7, 2023, an employee was driving an order picker down an aisle when the mast of the vehicle struck a material pan that was hanging out of the rack. The pan fell and struck the employee, resulting in rib fractures and a spleen laceration.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.
