An employee was pulling a truck from a loading dock but could not get enough traction under the tires due to ice. After pulling the truck forward approximately 2 feet, the employee exited the vehicle to retrieve ice melt located in the rear of the trailer. The employee then dropped the bag of ice melt to the ground behind the trailer and was standing at ground level between the trailer and the loading dock. The airbag in the rear tag axle of the trailer engaged, lifting the rear axle up off the ground. The truck rolled backward, pinning him against the building. The employee was hospitalized with multiple pelvic fractures and a crushing injury to the abdomen and lower back, which required multiple surgeries.
An employee was clearing a jam on a pre-form wheel. The employee's left hand became stuck between the wheel and a metal piece used to feed material, causing the amputation of the index finger.
An employee was training on an rolling container carrier on the dry, non-frozen storage side of the distribution center. She made a turn around the end cap and her right foot struck the rail protecting the endcap racking. The employee's foot was fractured.
An employee was a passenger in a utility vehicle that was driving up a hill on a wooded trail. As the vehicle made a right turn, three fingers on the employee's right hand were caught against a tree and fractured.
An employee was working next to a 12-inch, 21-foot-long pipe that was being lowered using a 1-ton chain fall. The chain fall slipped, causing the steel pipe to drop. It struck his left hand, causing the partial amputation of the ring fingertip and the loss of the fingernail.
Three employees were reorganizing a work area by staging material-handling tables. Two employees were pushing a table while the third employee guided it. The table became stuck on the wheel of another nearby table and while the two employees tried to push it free, the third employee moved the other table. The newly freed table moved forward into the other table, and the third employee's right middle finger was caught between the two. A third of the fingertip was amputated.
An employee was fabricating an evaporator fixture. The fixture was sitting on a table and the employee was utilizing a hoist and chain sling hooks to elevate one side of the fixture into position to weld a piece on it. The hoist moved slightly on the bridge trolley, creating slack in the chain that caused one of the legs of the sling to come out of the square tubing. The evaporator fixture came down onto the employee's right ring finger, pinning it against the table. The employee's finger was partially amputated and was later surgically amputated above the knuckle.
An employee was removing material from a pallet rack using a standup forklift when an upper shelf was knocked out of place causing a pallet of industrial fans to fall 12-15 feet. The employee was struck by the falling pallet. The employee sustained a laceration to the front-right of their head and fractures to three vertebrae.
An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.
An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.
An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
