An employee opened the drum and pail kettle to look inside. The cover fell in. The agitator spun the cover around and knocked the employee off the platform resulting in an approximate six foot fall.

On September 15, 2024, an employee was clearing a piece of paper on the heat sealer in the flour bagging area when they sustained a partial amputation of their left thumb at the first knuckle.

An employee was operating the bake sealer (used to seal the product) on the line and there a change over in the process. The employee was moving the bake sealer when he slipped and his hands were caught between the moving belts of it, resulting in a partial amputation of the left little finger.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was going down a ramp to a basement to assist in cleanup efforts. The ramp had been closed to vehicle traffic all day, as it was slippery. The employee was halfway down the ramp when a forklift drove onto it, then slid down the ramp and struck him. The employee suffered a spinal fracture and was hospitalized.

An employee was clearing a jam and making an adjustment to a machine using the exit wheel. Their right thumb was caught in the machine and they sustained a partial amputation to the thumb tip.

An employee was pushing a bag that was stuck in the bag sealing machine when their right thumb became caught in one of the back wheels, resulting in an amputation of the thumb tip.
