The oscillator's belt made a forward motion and the employee's left hand was pinched between the belt and the belt wheel. The employee suffered a hand laceration and amputation of the left middle finger to the first knuckle.

An employee was loading a reel using take-up equipment when the equipment amputated the employee's right pinky finger. The equipment was not guarded at the time.
