An employee, working as a coner, which is the person who sits on the platform crate/grate behind the road line marking paint truck and picks up and places traffic cones, was picking up cones from the right side of the crate/grate when the truck hit a bump causing it to bounce up and down which caused the employee's right leg to get stuck between the ground and the vehicle breaking the tibia/fibula.

An employee was driving a truck and hit a set of railroad tracks. The truck bounced and the employee hurt his back.

TKM FARMS, INC.

The injured employee and his coworkers were riding the bus to the jobsite. There was a dirt bump in the road and the driver lost control of the bus. The employee sustained a possible head injury and injuries to his right hip and right hand.

American Medical Response

A paramedic was riding in the back of an ambulance when it ran over a pothole or bump in the road, jarring his back. He suffered lower back pain and was hospitalized.

Honeywell International Inc.

An employee was driving a front-load/bucket tractor on an unpaved roadway. While surveying conditions, the employee stopped the tractor and positioned the front load bucket to approximately 18 inches above the ground to take pictures/video of a water-filled ravine that crossed the main roadway. The employee was then driving forward when the front wheels rolled down a slight sloped wash. The bucket embedded in the dirt, causing the tractor to stop. The employee's abdomen was injured from the abrupt stop and the pressure from the seatbelt he was wearing.

Portsmouth Naval Shipyard

On 6/25/15, at 10:30 a.m., an employee was riding her personal Segway on a street at the Navy base. While between buildings, she hit a dip or pothole in the roadway and lost control of her vehicle. She crashed her vehicle and was hospitalized overnight for a hip area fracture.

Cashless Tolling Constructors

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

Anderson Columbia Co., Inc.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Don Martin Corporation

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
