Final narrative
An employee was cutting pieces of plate using a band saw. The employee's glove got caught by the saw causing the amputation of a portion of his left thumb. The employee was taken to the hospital for treatment.
A forklift was used to load a flammable storage cabinet (weighing approximately 265 pounds) into the bed of a pickup truck. Once the forklift was removed and parked, employees went to re-position the cabinet. They lifted one edge of the cabinet to roll it on its side. The cabinet dropped approximately 2 inches and landed on the injured employee s left middle finger causing a fingertip amputation.
An employee was filling a sandblast pot with more media. As he stepped away from it, the hose fitting decoupled, leaving his right leg in line with blasting media discharge. The discharge caused an abrasion in the right curved area of his right leg.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
